The Power of PACE - All-inclusive care for aging in place

The seminar, "The Power of PACE: All-Inclusive Care for Aging in Place," presented by local experts from Summit ElderCare—Fallon Health, will provide a comprehensive overview of the Program of All-Inclusive Care for the Elderly (PACE). The program's core mission is to help frail seniors, aged 55 and older and certified as needing a nursing home level of care, remain safely in their community. The discussion will cover key eligibility criteria, the financial benefits of integrating Medicare and Medicaid with no copayments, and the central role of the Interdisciplinary Team (IDT) in coordinating all medical, social, and long-term care needs. Attendees will learn how the PACE Center serves as a hub for services, therapies, and socialization, ensuring a complete and integrated care plan, from prescription drugs and home care to specialty services and transportation.
